Access to Information and Privacy Request Forms
Forms are available to help you obtain the information you need.
Access to Information Request Form (TBC/CTC 350-57) – This form is used for requests for federal government information. Complete steps 1 through 4 to get the information you need.
Personal Information Request Form (TBC/CTC 350-58) – This form is used for requests for personal information that the federal government has about you, as established under the Privacy Act. Complete steps 1 through 4 to get the information you need.
Record Correction Request Form (TBS/SCT 350-11) – If you believe that there are errors or omissions in the information that the federal government has about you, use this form to request corrections. The completed form should be sent to the government institution that holds your personal information.
